Home Health Aide/Certified Nurse Aide to become part of not just a team

Seeking a paraprofessional person who is specifically trained,

competent and performs assigned functions of personal care to the

patient in their residence under the direction, instruction and

supervision of the Registered Nurse (RN).The duties of a Home Health

Aide include the provision of hands-on personal care, performance of

simple procedures as an extension of the therapy or nursing services,

assistance in ambulation or exercises, and assistance in administering

medications that are ordinarily self-administered.

Responsibilities:

Licensed Practical Nurse [LPN]

nurses to become part of not just a team but a family to service the

areas of: Brevard and Seminole County.

Job Summary:

A qualified Licensed Practical Nurse administers skilled nursing care

to patients on a intermittent basis in their place of residence. This

is performed in accordance with physician orders and plan of care under

the direction and supervision of the Registered Nurse. Services are

furnished in accordance with Agency policies.


Responsibilities:

1. Understands and adheres to established policies and procedures.

2. Implements the nursing care plan for each patient.

3. Provides nursing services, treatments, diagnostics and preventive procedures as assigned.

4. Initiates preventive and rehabilitative nursing procedures as appropriate.

5. Observes signs and symptoms and reports to the physician and RN

reactions to treatments, including drugs and changes in the patient’s

physical or emotional condition.

6. Teaches and counsels the patient and family/significant others

regarding the nursing care needs and other related problems of the

patient at home.

7. Evaluates with registered nurse the effectiveness of the LPN’s

nursing service to the patient and family under the guidance of the

registered nurse.

8. Maintains accurate and complete records of observations, treatments and care of patient.

9. Participates in medical record audit as assigned.

10. Attends staff meetings, patient care conferences and in-services as scheduled.

11. Task on-call duty, nights, weekends and holidays as assigned.

12. Responsible for:

A) Submitting any changes in schedule to the Director of Nursing/Nursing Supervisor on a daily basis.

B) Participating in-patient care conferences to discuss the need for

involvement of other members of the health team, such as physical

therapist or speech language pathologist.

13. Prepares clinical and progress notes.

14. Assists the physician and RN in performing specialized procedures.

15. Prepares equipment and materials for treatments.

16. Observes aseptic technique as required.

17. Assist the patient in learning appropriate self-care techniques.

18. Maintains confidentiality and ensures HIPAA guidelines and procedures are followed.


Qualifications:

Graduate of a State approval school of practical (vocational)

nursing and currently licensed in the State(s) in which practicing.

Minimum of one (1) year experience in nursing (Home Health Care/Community/Hospital settings), required.

Complete knowledge of nursing principles and procedures of skills and the techniques of good patient care.

Experience with IV infusion, wound care and ventilators.

Acceptance of philosophy and goals of the Agency.

Ability to exercise initiative and independent judgment.

Basic computer skills.

Excellent and effective communication skills.

Must be able to speak, read and write English effectively.

Flexible Schedule.

Time Management and organizational skills.

Bilingual (Spanish), a plus.

Registered Nurse [RN]

The Registered Nurse (RN) is Responsible to evaluate/assess, plan,

implement, coordinate and monitor patient and family needs in order to

promote the highest quality of care. RN will case manage and be

responsible for patients assigned and direct all other disciplines with

proper plan of care. Promote education and training of properly use of

equipment and perform procedures at home. Support patient/family to

follow medication regimens and rehabilitation program. Communicate

progress to Physician and encourage compliance with medical

appointments. Decrease the chance of hospitalizations and emergency room

visits.

Reports to: Clinical Supervisor / Director of Nursing

 

Job Responsibilities and Functions:

* Completes initial and ongoing comprehensive health and

psychosocial assessment to determine the qualifications of home care

needs and homebound status.

* Communicates?Reports to clinical Supervisor/Director of nursing periodically of changes and conditions.

* Communicate with attending physician regarding plan of care and any change in status.

* Responsible for performing admission ans completing SOC Orders and

OASIS, Transfers, Recertification, and DC oasis, and HHA plan of care

paperwork.

* Reviews with clients medications, pain Management, Safety precautions.

* Collaborates with Clinical Supervisor/Director of nursing if any

changes in status and make proper modifications to plan of care.

* Attends case Conference as needed.

* Performs supervisory visits to supervise LPN’s and HHA.

* Completes all Documentation Legible and within the time frame incorporated by the agency.

* Maintains confidentiality and ensures HIPPAA guidelines and procedures.

* Demonstrate proper safety and infection control practice, use of PPE, and and Hygiene.

* Maintains daily Attendance log in client’s home.

community, assists with activities of daily living if needed, and

DSP may provide supports to a person with a disability at home, work,

school, church, and other community places. A DSP also acts as an

advocate for the disabled individual, in communicating their needs,

Duties and Responsibilities:

• Assist with daily living skills (e.g., lifting, transferring, toileting, personal hygiene, laundry, meal preparation).

• Promote appropriate community integration and social interaction (e.g. visiting family, friends, church/temple, etc).

• Implement outcomes developed through a Support Plan.

• Complete paperwork as assigned including documentation (e.g., incident reports, outcomes, log entries).

• Assist the consumer with recreational, cultural and vacation plans of his/her choice.

• Provide independent and functional living instructions to the consumers.

Education and Experience:

• High School Diploma or G.E.D. Required.

• Must have valid driver’s license and a clean driving record.

• Must be able to work independently.
